sizes. The information you provide helps your leasing consultant
find a home that best fits your family's needs.

Active Duty Applicants
Important Information for Active Duty Service Members
Required Documents for Active Duty Service
Members:
- Official Orders
- Most Current Leave & Earnings Statement (LES)
- DDS Form-1172 (if all dependents are not listed on Official Orders)
- Completed Form AF-4422 (Each applicant over the age of 18 must complete and submit this form)
- Power of Attorney (POA) if service member is unable to sign and date application
- Completed Privacy Information Release Form
- If applicable - Photo of pet(s) and vaccination records
To be placed on the wait list, a copy
of the service member's orders must accompany the online
application. In addition, please review the list above for
documents you can submit to get your application processed
faster.
Please note: Some military server firewalls block the application
process, if possible, please use a personal computer, tablet or
smartphone.
Other Eligible Tenants
Important Information for Other Eligible Tenant Applicants
Please upload the following documents with your online
application:
- Proof of Income (2 months of pay stubs per applicant and per employer)
- Completed Form AF-4422 (Each applicant over the age of 18 must complete and submit this form)
- Completed NCIC Consent (Each applicant over the age of 18 must complete and submit this form)
- Completed Tenant Consent (Each applicant over the age of 18 must complete and submit this form)
- Completed Privacy Information Release Form
- Retired Military must provide DD214 and Retirement Leave & Earnings Statements with their application.
- If applicable - Photo of pet(s) and vaccination records
